Appointments

Visits are available by appointment only. Same-day visits and visits within 24 hours are reserved for acute care needs covered by one or more physicians. We are unable to serve walk-in appointments.
Please help us keep appointment schedules by arriving 15 minutes early to check in and complete any updates we may need.
You must cancel at least 24 hours in advance, or we will charge a $50.00 no-show fee. Patients arriving more than 10 minutes late for their scheduled appointment may be subject to rescheduling and charges, based on the availability of the provider.
Please bring your current insurance card, a valid identification card, and a list of your current medications, including any over-the-counter or natural supplements that you are taking. Please inform us if you have been seen by another provider.
We appreciate your understanding that our physicians occasionally fall behind schedule due to urgent appointments or complicated cases, we will keep you informed of our schedule upon arrival if we are running behind and give you the option to reschedule.
Cancellations
Please notify us of your need to cancel an appointment at least 24 hours in advance of the appointment. You may cancel any time 24/7 by leaving a telephone message or online through our patient portal. A no-show fee of $50.00 may be charged if you do not show for your appointment without such notification.
Payments
Co-payments are due at the time of the visit, and we cannot bill you for payment later. Should we have to generate a bill, a late charge of $25.00 will apply. Statements will be generated immediately upon settlement of insurance billing. A second courtesy statement will be sent approximately 30 days later. Payments of unpaid balance are due upon receipt of statement. If payment is not received within 10 days of the second statement, your account will be considered delinquent and a late fee of $25.00 will be applied. Accounts that have not been settled within 60 days of the initial statement may be referred to an outside credit agency for collection.
Cash Discounts
We offer cash discounts for our uninsured patients with full payment due at the time of service. Cash rates are only available for our non-insured patients. Due to insurance contracts, we are unable to discount our contracted rates or patient-allocated balances. Discounts are also available on laboratory services through Quest Diagnostics. Please ask for details at the front desk or ask to speak to the manager.
Insurance Billing
We accept a wide variety of insurance plans including Sharp Community Medical Group HMOs (with the exception of Dr. Sclar), Medicare, Blue Cross Blue Shield PPOs, Aetna PPOs, United Health Care PPOs, Tri-West, and many more. To ensure the most up-to-date in-network information, please call your health insurance plan or visit their web site to view their current providers. Any balances due by the patient will be billed as above.
If you are a new patient looking for care, please contact our office to discuss your insurance and provider options as each provider is contracted separately. We do not accept Worker’s Compensation or Third-Party Liability insurances such as motor vehicle accident claims.
Prescription Requests
Please allow a minimum of 72 hours for all prescription refills. New prescription requests or antibiotics will require an appointment. Refills may be requested by contacting your pharmacy or by sending us a secure refill request through our patient portal. Mail order and military base prescription refill requests: these refill requests may be called in or sent via secure message though our Patient Portal. Please allow 72 hours for processing of your request; our staff will notify you when your prescription is ready for pick up.
Only urgent cardiac medication refill requests will be authorized after hours.
